At Hands-On Health, we believe your health is your wealth, and our Wealthy Heart program empowers individuals and families, from pregnancy and postpartum to school-age children to older adults, to protect their heart’s wealth through practical, hands-on heart-health education.
A curriculum created by our co-founding pharmacists, the Wealthy Heart program promotes nutrition, mindfulness, and movement as medicine, offering simple, culturally respectful tools that help prevent heart disease. Our mobile programs and team of diverse medical professionals and volunteers bring heart health screenings, Mediterranean diet and plant-based cooking demonstrations, and interactive wellness activities directly to where participants already worship, learn, work, and spend leisure time such as schools, workplaces, recreation centers, and faith communities.
We meet every participant where they are, mentally and physically, reducing barriers to care and addressing social determinants of health through trust, accessibility, and community connection. Each session blends education with action: learning how nutrition supports heart health, practicing cooking strategies on a SNAP budget, understanding blood pressure and glucose readings, and engaging in achievable, enjoyable physical movement that fits into daily routines.
While we center on a whole-food, Mediterranean diet, plant-based nutrition, we honor each family’s unique food journey. We take a practical, culturally respectful, plant-based, and plant-forward approach that meets participants exactly where they are in their readiness for change. Our focus is small steps, not drastic, uncomfortable changes. Families who rely on processed foods learn how to add more vegetables, try meat alternatives, choose whole grains, reduce processed foods, and make small, sustainable shifts that feel achievable and supportive of their cultural traditions.
For example, our Burgers and Blood Pressures annual program recruiting events demonstrate this philosophy in action. Participants receive complimentary veggie burgers topped with whole-food ingredients like avocado, lettuce, tomato, and onion, showing how heart-healthy, plant-based meals can be satisfying and delicious.
For those not yet enthusiastic about veggie burgers, we offer a turkey or chicken burger paired with the same toppings, helping them take an approachable, plant-forward step without pressure or judgment.
Our goal is to support realistic, sustainable change in communities disproportionately affected by contributing factors of heart disease. By combining whole-food, plant-based education with cultural humility, intergenerational engagement, and convenient delivery in familiar community settings, we empower individuals and families to take meaningful steps toward heart health—at their own pace.

FAMILY DATE DAY
Family Date Day is an intersection that merges the Health Science Labs (youth) with our Wealthy Heart Program ( adults and families) to create an intergenerational learning atmosphere for families to practice optimum nutrition, positive thinking, and healthy movements, together!
